OBTAIN TELEPHONE ADVICE?

Doctors try to be available for telephone consultations between 12.00 and 12.30pm weekdays (tel: 8946 4820). For medical advice at other times, phone NHS Direct on 0845 4647.

The practice nurse is also available for telephone consultations between 12.15 - 12.45pm weekdays.

MAKE APPOINTMENTS?

Our usual appointment time is 10 minutes. Please make one appointment for one person about one problem. If you are unable to keep your appointment please telephone to cancel. Failure to do so means a wasted appointment that another patient could have used.

• Consultations with the doctors and nurses are by appointment.

• Consultations with the phlebotomist are also by appointment; the doctor will ask you to book this when he refers you for a blood test.

OBTAIN HOME VISITS?

Visits are usually restricted to terminally ill or housebound patients.
Please request a home visit before 10.00am, except in an emergency.

CHANGE MY DETAILS?

Please let us know of any changes to your name, address or telephone number so that your records may be kept up to date.

OBTAIN REPEAT PRESCRIPTIONS?

Please allow at least 72 hours (three working days) to process prescriptions. Please use your prescription counterfoil to order a new prescription. Tick the items you require and post or fax it to the surgery (fax number 020 8944 9794).

No telephone requests will be accepted.

HOW DO I...
REGISTER AS A PATIENT?

To register at this practice please check that you live within our catchment area. You will be asked to complete a registration form and a questionnaire outlining your medical history. You will also need to provide proof of address and a urine sample for each member of the family. Please telephone after 48 hours to confirm that you have been accepted for NHS treatment and that your details have been added to our computer database.
