MAKE APPOINTMENTS?
Our usual appointment time is 10 minutes. Please make one appointment for one person about one problem. If you are unable to keep your appointment please telephone to cancel. Failure to do so means a wasted appointment that another patient could have used.
Consultations with the doctors
and nurses are by appointment.
Consultations with the phlebotomist
are by appointment, the doctor having requested the patient to make an appointment.

Please request a home visit before 10.00am, except in an emergency.

to your name, address or telephone number so that your records may be kept up
to date.

OBTAIN REPEAT PRESCRIPTIONS?
Please allow at least 72 hours (three working days)
to process prescriptions. Please use your prescription counterfoil to order a
new prescription. Tick the items you require and post or fax it to the surgery
(fax number 020 8944 9794).
No telephone requests will be accepted.

REGISTER AS A PATIENT?
To register at this practice please
check that you live within our catchment area (see map). You will be asked
to complete a registration form and a questionnaire outlining your medical history.
You will also need to provide proof of address and a urine sample for each member of the family. Please
telephone after 48 hours to confirm that you have been accepted for NHS treatment
and that your details have been added to our computer database.
